In Practice

A prescriber writing notes at a desk

In short

In practice, the prescriber adds three or four words to each repeat prescription, using the existing prescribing software, and the pharmacist records the indication with the directions. Highfield Surgery reached 85% of repeat prescriptions, and departure and destination audits check that the indication travels with the medicine from surgery to home.

Questions

Highfield Surgery, the pilot practice (5,800 patients), added the indication as each repeat request came in and took one and a half years to reach 85% coverage. Today the indication is added as free text in the directions, and in EMIS it is then offered from a pick list (FAQ 3).

With departure and destination audits: a check of repeat scripts where they are produced, against a standard of 80%, and a check of the medicines at their destination, such as a residential home.

In one practice where 85% of repeat prescriptions carried an indication, average statin compliance over a year was 93%. The authors say prospective studies with controls are needed to show whether indications improve adherence (the compliance study).
